Referee Forms
Availability Form - Used by Norwood Referees to provide game availability. If you are an official NYS referee, you should complete this form and return it to the Referee coordinator Peter Varitimos.
Service Agreement - As a referee for the town of Norwood, you must complete the Service Agreement form. This form provides the most accurate payment details from Norwood. It also serves as a contract which details what is expected of you as an NYS Referee. You should sign and comnplete this form before each season.
